Applications of Carbapenems

Carbapenems are a class of broad-spectrum antibiotics that are primarily used to treat serious infections caused by bacteria that are resistant to other types of antibiotics. Some common applications of carbapenems include:

Mechanism of Action of Carbapenems
As like other β- Lactam antibiotics, the mechanism of action of carbapenems involves disrupting bacterial cell wall synthesis and causing cell death through inhibition of peptidoglycan cross-linking. Specifically, carbapenems exert their bactericidal effect by binding to penicillin-binding proteins (PBPs) (also known as DD-transpeptidases) in the bacterial cell wall, which are enzymes involved in the cross-linking of peptidoglycan chains. This binding inhibits the transpeptidation reaction that is essential for proper cell wall synthesis, leading to cell lysis and death of the bacteria. On the other hand, the inhibition of transpeptidation and crosslinking causes the buildup of new peptidoglycans. This leads to the activation of autolytic hydrolases, which break down the current peptidoglycan layer without replacing it, ultimately further increasing the bactericidal impact.
